Intake Process

This office utilizes electronic mental health records, and all of your information is stored digitally. There's no paper to waste, and all information is at your fingertips!

You will be sent a link by email that takes you to your personal client portal. There you can fill out all forms needed to begin the therapy journey at our office. The office manager will be alerted when the forms are completed, and your appointment will then be confirmed.  Your therapist will review your paperwork prior to your first meeting.

Should you need to update any of the information, simply alert the office manager and changes can be made immediately:
